Clinical Pharmacist II - Remote Order Verification Team

Location: Toledo Hospital Department: Pharmacy Remote Order Verification Weekly Hours: 24 Status: Part time Shift: Variable (reputed company of America) Job reputed company: ACCOUNTABILITIES *reputed company duties listed below are essential unless noted otherwise* Clinical Pharmacy reputed company Performs certain duties of Clinical Pharmacist I, including medication order verification review and respond to critical lab values. Reviews targeted patient reputed company and recommends evidence-based medication therapy regimens and monitoring plans, ensuring cost-effective therapeutic alternatives are considered. Monitors medication therapy regimens for contraindications, interactions, adverse reactions, appropriate dosing, and therapeutic duplications. Contributes to antimicrobial stewardship activities, ensuring appropriate use of antibiotics through medication order verification. Participates in anticoagulant stewardship to enhance safety and efficacy of therapy through medication order verification. Verifies chemotherapy and parenteral nutrition orders. Verifies pediatric orders with precision and clinical appropriateness. Drug Information and Escalation Assists in addressing drug information requests escalated from Clinical Pharmacist I relevant to medication order verification and appropriately escalates more reputed company issues, as necessary. Provides guidance in evaluating evidence-based literature to support drug therapy reputed company. Supervision, reputed company, and Training Assists in training and mentoring pharmacy staff in medication order verification, fostering reputed company development and clinical reputed company. SCHEDULE Training in the Toledo office until fully acclimated to the role 8 hr- shifts can work between 6am - 10pm mostly evening schedules rotating every reputed company weekend 2 rotating Holidays a year REQUIRED QUALIFICATIONS Education: PharmD degree or BS Pharm with relevant experience post- 2006. Skills: Experience in hospital pharmacy services, including unit reputed company, IV admixtures, medication reconciliation, and addressing provider questions. Years of Experience: Minimum 1 year (full-time equivalent of 2,040 hours) in inpatient pharmacy reputed company. License: License: Must be a registered pharmacist, licensed in good standing in Michigan and Ohio. Single facility-based pharmacists must retain state licensure per site location. Certification: BLS (Basic Life Support) PREFERRED QUALIFICATIONS Education: Completion of a PGY1 Pharmacy Residency Training Program. Skills: Expertise in patient counseling, pharmacokinetic evaluations, preceptorship, antimicrobial stewardship basics, evidence-based literature review, and time management. Experience in multidisciplinary team collaboration is highly desirable. Years of Experience: Minimum 3 years (full-time equivalent) in inpatient pharmacy reputed company. Certification: ACLS (Advanced Cardiac Life Support), reputed company Certification, or two reputed company training certificate programs for acute care. WORKING CONDITIONS Physical Demands: Must be reputed company to lift light materials.
