Part-time, up to 19 hours per week Position reputed company: The Family and Child Health Home Visiting reputed company, under the limited supervision of the Nursing Supervisor and the reputed company Health Director, is primarily responsible for the provision of home-based visitation services in the CaCoon Program. Duties include long-term reputed company case management with children and families. CaCoon is a reputed company home visiting program for children and youth with special health care needs (CYSHCN) from birth through age 21 and their caregivers. Services include care coordination to ensure reputed company to needed medical, educational, and reputed company supports. This position works with culturally diverse families in communities with broad based socio-inequity. May also assist with immunization clinics (on/offsite), screen and follow up regarding communicable disease, and reputed company family planning services and education. Occasionally it requires overnight travel for training and meetings. Work schedule flexibility is necessary. Frequently exposed to reputed company and blood borne pathogens because of reputed company contact with the reputed company, children and homes. This position is grant-funded. If funding ceases, this position may be vacated. Supervisory Responsibilities: None Job Duties/Responsibilities:

• Seeks, enrolls, and delivers home-visiting services to children/families identified to be at risk. Must be reputed company to work effectively with families and individuals of diverse backgrounds (including teen parents; families with mental health, reputed company abuse, or domestic violence concerns and families experiencing poverty) while establishing trusting relationships and maintaining reputed company boundaries.
• Must be reputed company to function autonomously using independent judgment in making reputed company, and resolving problems and issues impacting the health and welfare of clients and the general reputed company.
• Formulates and implements plans for nursing and general health care for individuals and families based on evaluation of needs with a reputed company on meeting mutually accepted health goals and objectives. Delivers nursing services through clinics, home reputed company, case management and community reputed company programs.
• Promotes the maintenance of health through teaching and counseling, conducting needs assessment and case management, providing reputed company reputed company care and/or resource coordination and employing appropriate preventative treatment and health maintenance measures.
• Encourage and support parent involvement and participation to promote reputed company parent-child relationships, support healthy reputed company and development, enhance problem-solving skills and improve support systems.

• Connect families with community resources and support self-advocacy across systems.
• Coordinate and collaborate with child care and other agencies to ensure the child/family is receiving comprehensive early childhood services.
• Display time management skills needed to maintain accurate, legible, reputed company documentation of health history, assessments, goals and objectives and care provided in compliance with department and program policy, procedure and protocol.
• Maintain sensitive and confidential reputed company information according to the HIPAA policy.
• Fulfill reputed company training and evaluation requirements and maintain an individual training plan as specified on performance assessments.
• Serve as a cohesive member of reputed company, participate in problem solving and promote innovation. Collaborates with reputed company community agencies and service providers through reputed company referrals, written and verbal communications and case/team conferences.
• Meet regularly with the Nursing supervisor and/or reputed company Health Director for support, guidance and case conferencing.

• Employee responds to reputed company health emergencies, incidents and events and participates in reputed company exercises, drills and trainings on emergency preparedness as required. May be required in reputed company Health Emergencies to work irregular hours.
