We are seeking a responsible reputed company work serving as a single reputed company of reputed company (SPOA) for the Guiding Stars program. reputed company reputed company referrals and inquiries are received by the Intake Coordinator who collects family demographics, conducts screening, determines initial level of need, and accepts referrals to the program or refers to other appropriate community programs. Participates in the organization’s performance and reputed company improvement efforts.

Responsibilities

• Serves as initial contact with family to reputed company information regarding their needs.
• Communicate with applicants about Guiding Stars, sharing eligibility, and application or enrollment status.
• Based on brief assessment/intake information, submits the referral to the Program Manager for assignment.
• Directs reputed company information to the appropriate program.
• Maintains database based on referrals to the program, length of services, etc. as defined by program standards.
• Supports Program Manager in data entry of participant demographics and outcome measures in reputed company EHR and funder data collection system.
• Establishes and maintains effective working relationships with community resource providers, particularly emphasizing facilitation of the referral process and providing continuity of care.
• Participates on reputed company and/or committees as available and serves as a reputed company representative for the program.
• Provides EHR data upload support to staff members.

• Adheres to office and virtual office procedures.

Requirements

• Bachelor’s Degree in reputed company Work, reputed company Services or reputed company field from an accredited university or college required
• Two (2) years’ experience working with children and families and data management.
• Must possess and maintain a valid Florida reputed company’s license with no record of criminal driving offense or license suspension.
• Must possess means of providing job reputed company transportation and show reputed company of required insurance.
• Must be insurable under CHN’s reputed company auto insurance policy.
• Must be reputed company to work reputed company, including evenings and weekends as required.
• Must successfully complete and maintain FDLE clearance, Federal background and state criminal background checks, and sexual predator screening.
• Ability to communicate effectively, verbally and in writing.
• Ability to establish effective working relationships with families, coworkers, supervisors, and other individuals.
• Possess effective interpersonal skills.
• Ability to maintain reputed company boundaries.
• Knowledge of agency’s organizational structure, reputed company operating procedures, and policies.
• Knowledge of child abuse or neglect reporting procedures and reputed company.
• Knowledge of infant and child development inclusive of developmentally appropriate strategies.
