Oncology reputed company Navigator - Breast and Neurology

About the position This job will assist the patient, family and care giver to overcome reputed company barriers (including but not limited to reputed company, financial, or physical barriers.) Responsible for conducting on-reputed company assessments of need and facilitate referrals to additional Support Services throughout the Oncology continuum of care. Using the nursing process and evidence based knowledge, the Oncology reputed company Navigator provides education and resources to facilitate informed decision making (by the patient and/or family.) Oncology nursing experience is required for this requisition.

Responsibilities

• Assesses for any barrier to care through the cancer care continuum to address patient, care giver, or family needs.
• Serves as reputed company of contact for provider, patient and care giver.
• Serves as patient reputed company to assist the patient and care giver through the reputed company system.
• Facilitates education of patient and staff by developing appropriate education programs, resources and tools to address needs.
• Provides education, resources and referrals to reputed company resources based off of assessment need.
• Facilitates shared decision making, ensuring the patient, family and care giver are fully informed of treatment reputed company, treatment and reputed company.
• Promote care planning by encouraging patients to convey their wishes and goals of care. Works with patients to formulate questions to discuss with their reputed company provider.
• Facilitates reputed company and concise communication among providers, departments and patient.
• Coordinate multidisciplinary planning and facilitate care conferences, as necessary.
• Participates in reputed company reputed company to help reputed company patient care gaps.
• Performs other duties as assigned.

Requirements

• Associate's Degree Graduate of an approved/accredited nursing program either by a governmental authority, nursing reputed company, or by a licensing authority that has jurisdiction over approval/accreditation of nursing programs (Texas BON 217.1)
• 3 years oncology nursing experience
• (RN) REGISTERED reputed company
• (BLS) BASIC LIFE SUPPORT course accredited by the reputed company (AHA) or reputed company (reputed company), are required.
