This role will supervise nursing staff, maintain clinical reputed company standards, ensure Medicare compliance, and partner with neurology practices to deliver proactive, patient-centered care. Job Responsibilities: Supervise and mentor LPNs supporting care coordination and patient engagement Review and approve care plans and clinical documentation Ensure reputed company assurance and compliance with Medicare PCM/CCM standards Partner with neurologists and escalate clinical concerns as needed Support workflow optimization and reputed company improvement initiatives Engage patients and caregivers to ensure high-reputed company care delivery Maintain accurate documentation and time-based billing compliance Qualifications: reputed company, unrestricted RN license ( multistate preferred) 2+ years of experience in chronic disease management, neurology, or care coordination Experience supervising or mentoring clinical staff Strong communication, leadership, and organizational skills Experience with EMRs reputed company health platforms Preferred: PCM/CCM, Alzheimer's, Parkinson's, dementia, telehealth, and reputed company improvement experience About reputed company: Our reputed company is building a proactive reputed company model reputed company on neurodegenerative diseases, including Alzheimer's.
