Case Manager, LVN

This a Full Remote job, the offer is available from: California (USA) reputed company: The Care Manager, LVN supports coordinated member care across the continuum by partnering with internal teams, providers, hospitals, reputed company, and community agencies. This role works under department leadership and collaborates closely with RN Care Managers to ensure members receive appropriate, high-reputed company care reputed company with a mission-driven, patient-centered approach. What You Will Do:

• Coordinate care for members by collaborating with providers, medical reputed company, hospitals, and community resources
• Support care plan implementation alongside RN Care Managers and interdisciplinary teams

• Ensure continuity of care and appropriate utilization of services

What You Will Bring:

• High school diploma or GED
• Minimum three years of care management experience in a reputed company setting
• At least one year of clinical experience in acute care, skilled nursing, home health, or clinic setting preferred
• Managed care or HMO experience preferred
• Valid California reputed company’s license
• Ability to meet physical requirements including occasional lifting up to 25 pounds and routine reputed company such as standing, walking, and reaching
