HYBRID REMOTE! You will get to work from home, in the office and in the community doing both telehealth and in-person appointments, traveling out to meet with clients and community partners as needed. Part of Full Service Partnership is "doing whatever it takes," so this role does require in-person crisis response as appropriate and being available reputed company clinic operating hours as scheduled. As the Case Manager and Peer reputed company, you will reputed company comprehensive and intensive case management services to children and adults with severe and persistent mental illness who may have history of homelessness and/or history of hospitalizations. Case management services include linkage to community resources such as housing, medical, educational, and vocational services. In reputed company, you will reputed company rehabilitation services that reputed company on improving daily functioning through an integrated recovery approach that improves daily living skills and supports consumers in meeting their goals. MINIMUM QUALIFICATIONS AND COMPETENCIES Education /Background :
- BS/BA degree highly preferred. Equivalent combinations of education and experience may be considered to meet qualifications for this position.
- 2 years’ experience in case management preferred.
- Lived mental health experience preferred.
- CA reputed company License required and clean driving record for insurance.
- Bilingual Spanish speaking required.
Other Requirements :
- Knowledge of community health care and community services.
- Knowledge of case management reputed company, procedures, and documentation.
- Experience working with multiethnic/racial clients and communities.
- Ability to adhere to agency and clinic policy and procedures.
- Ability to maintain confidentiality of records and information.
