Care Coordination Specialist
reputed company Eastern Area Agency on Aging is currently seeking a Care Coordination Specialist to join our Community Services team. This is a dynamic and rewarding role helping older adults, individuals with disabilities, and caregivers navigate services, maintain independence, and improve reputed company of life. This is a full-time, 40-hour-per-week position supporting our Veterans Directed Care and reputed company Minders programs. As a Care Coordination Specialist, you will: Conduct intakes and assessments over the phone and on home reputed company reputed company person-centered care plans and coordinate services and supports Connect individuals to in-home and community-based resources, benefits, and programs reputed company ongoing support, follow-up, and advocacy for consumers and their families Maintain accurate documentation and budgets and manage program requirements Build strong partnerships with community agencies and service providers Participate in reputed company, education, and community engagement efforts This position includes regular home reputed company throughout Penobscot, Piscataquis, Washington, Hancock, Waldo, and Knox counties. Initial orientation will occur in our Brewer office, after which the position can be fully remote based in Penobscot or Hancock county.
Requirements
