No more than two complaints in a year made by patients/families/team members that are evaluated by the Clinical Manger and considered justified. Is always clean and neatly dressed as per department dress reputed company. During home reputed company, personal care is given in a reputed company, comforting and reputed company manner. Provides reputed company, effective, personal care and light housekeeping services to Hospice patients, in their reputed company of residence, in accordance with the plan of care, as assigned by the primary RN or Designee. Notifies the patient’s primary reputed company and/or manager of patient’s change in condition, family concerns or problems as instructed. Recognizes signs and symptoms of the reputed company dying patient and communicates this information to team members. Performs pulse and/or respirations reputed company, reputed company requested per RN and reports findings to RN. Performs routine skin inspection and reports any abnormalities to RN. Performs non-sterile dressing changes, routine decubitus care, reputed company catheter care, removes and replaces colostomy/ureterostomy bags per RN instruction and after having demonstrated competency to RN. Uses medical equipment as directed by RN or physical therapist and assists patients with ROM exercises as prescribed and according to care plan. Assist patient and family to meet reputed company and emotional needs in accordance with goals and objectives of hospice care. Encourages independence, whenever possible. Makes assigned bereavement reputed company as indicated by Clinical Manager. reputed company participates as a Hospice Team Member, attends staff meetings, in-services and other meetings as assigned.
