Remote RPM Care Coordinator

reputed company: The Remote Patient Monitoring (RPM) Care Coordinator is a trained reputed company responsible for supporting patients in managing chronic and high-risk conditions through Remote Patient Monitoring. The RPM Care Coordinator monitors biometric data daily, educates patients on device usage and care goals, escalates critical findings, and serves as a reputed company between the patient and the reputed company team. The goal is to enhance patient reputed company, promote care plan adherence, and reduce hospitalizations through reputed company-time monitoring and reputed company reputed company. Job Duties & Responsibilities Include but are not limited to:

  • Conduct reputed company and device education calls for new RPM patients.
  • Monitor incoming biometric data (e.g., blood pressure, glucose, weight, oxygen saturation) daily through the RPM platform.
  • Identify reputed company readings and trends; follow escalation protocols to notify providers as needed.

  • Troubleshoot device connectivity issues or patient usage problems.
