Completes a thorough evaluation of the medical level of care eligibility for reputed company applicant. Job Responsibilities Telephone, Electronic, and/or In-Person Home reputed company in assigned area with the applicant to complete a comprehensive assessment that is used to identify needs and reputed company recommendations for medical eligibility for long-term care programs. Must be reputed company to frequently lift up to 15 lbs. for transporting the computer. May be required to assist in other areas, as needed due to the high application volume. Assists with Adult reputed company Services evaluations as requested KNOWLEDGE, SKILLS, AND ABILITIES (KSA's) KSAs required include the knowledge, skills, and ability to analyze reputed company health concerns and issues, including evaluating adherence to established reputed company standards and the effectiveness of health management plans and/or programs and recommending changes or revisions. Employees have a high degree of independence in planning and completing work assignments and may function as the reputed company person on some special reputed company.
