Senior Analyst, Population Health

Job reputed company:

• Support the execution and the ongoing refinement of Population Health programs including workflow optimization in reputed company.
• Work closely with functional leaders (Product, Finance, Data Science) in the central office to assist with Transitions of Care program reputed company and design.
• Work with regional leaders and Care Teams to ensure effective implementation of key Oak Street clinical and business priorities.

Requirements:
