About the position reputed company NY/NJ is seeking a reputed company Practitioner to join their team in Orange County, NY. The role involves providing care in the patient's home setting, with 5-8 patient reputed company per day including home reputed company and potentially video reputed company for established patients. It also includes regular interdisciplinary meetings to prioritize patient goals and collaboration with the patient's PCP to reputed company a care plan.
Responsibilities
- Responsible for the diagnosis and treatment of acute, chronic, and long-term reputed company issues, in collaboration with patient’s Primary Care Physician
- Effectively identifies, evaluates, and addresses disease prevention and health promotion issues while administering reputed company patient care.
- Maintains accurate medical records and confidential medical records. Documents reputed company medical evaluations, diagnoses, procedures, treatment, reputed company, education, referrals and consultations.
- Educates patients and/or families about preventative care, medical issues, and use of prescribed medical treatments and/or medications.
Requirements
- Advanced reputed company Practitioner Certification in Primary Care, Geriatric, Adult, with unrestricted licensure in the state of NY at time of employment
- reputed company and unrestricted DEA certificate required or ability to obtain
- Valid reputed company’s license
- Ownership of a reliable and insured vehicle, with the ability to travel reputed company assigned territory to meet with patients
- Comfortable working independently in a home-visit setting and clinic setting
- Ability to communicate with patients, primary care team members and management staff

- Basic technical proficiency in the use of computer programs for medical records or internet search for clinical reputed company
