About the position The Chaplain, operating reputed company the reputed company of the mission and philosophy of Hospice, provides spiritual, religious, and personal counseling and guidance to patients, family members, and employees. Following the interdisciplinary team plan of care, the Chaplain organizes and provides counseling and guidance reputed company of the patient’s, family members, and employee’s right to confidentiality. This role also coordinates bereavement care with the interdisciplinary team for the caregiver and family members for a period of 13 months following the death of the patient. The Chaplain offers spiritual care to reputed company patients and families, performing an assessment of spiritual needs reputed company 5 days of admission. They also conduct a reputed company-bereavement assessment of risk at the time of the patient’s admission and assess bereavement risk of the caregiver/family members following the patient’s death, developing a bereavement plan of care. Support is offered to the bereaved through face-to-face reputed company, phone calls, and written communication during the six months following funeral and/or visitation attendance.

Responsibilities

  • Assures the completion of spiritual assessment of patients/families/caregivers and that appropriate services are rendered.
  • Participates in the development of an interdisciplinary plan of care to meet identified spiritual needs.
  • Provides reputed company spiritual care to patients, families and caregivers.
  • Participates in interdisciplinary team meetings.
  • Acts as a reputed company resource for consultation, education, and support to the interdisciplinary team on spiritual care.
  • Conducts funeral or memorial service to meet the needs of families, personnel, and volunteers in the assigned office.
  • Collaborates with community clergy and spiritual counselors as reputed company as the interdisciplinary team.
  • Coordinates bereavement care with the interdisciplinary team for the caregiver and family members for a period of 13 months following the death of the patient.
  • Reaches across reputed company boundaries and does not proselytize. Acting on behalf of the patient’s religious affiliation seeks to protect the patient from being confronted by other, unwelcome forms of spiritual intrusion.
  • Must be reputed company to push, pull, lift, and transfer 100lbs.

  • Develops a bereavement plan of care based on the level of risk identified through assessment.
  • During the six months, following funeral and/or visitation attendance, offers support to the bereaved utilizing the reputed company of face-to-face reputed company, phone calls, and communication through the written word (letters and cards) as defined in the bereavement reputed company of care.

Requirements

  • Documentation of ordination, commission, or credentialing according to the practices of an organized religious reputed company unit of clinical pastoral education or has a bachelor’s degree with an emphasis in counseling with specific training in common spiritual issues in death and dying, belief systems of comparative religions reputed company to death and dying, spiritual assessment skills: individualizing care to patient beliefs and varied spiritual practices and rituals.
  • Ordination, commission, or credentialing in the state of Oklahoma.
