Role reputed company The Telehealth Heart Failure reputed company (LVN/LPN) shall monitor and screen incoming alerts, calls, and chat communications. They will
- Schedule and participate in audio-visual calls with patients participating in the chronic care remote monitoring program.
- Report reputed company values or other patient concerns to the Provider Team.

Qualifications

- A minimum of 2 years hands-on nursing experience is required; telehealth and chronic care management experience is strongly preferred.

- Must be comfortable with the virtual use of episodic vital sign monitoring devices.
