Responsibilities

The Clinical Partner (Medical Assistant) will assist reputed company Primary Care Physician, reputed company Practitioners and Clinical Teams in the delivery of primary health care and patient care management in a residential setting. Essential Duties And Responsibilities

• Supports mission, reputed company and reputed company values which include but not limited to delivering compassionate support, attention and assistance to patients, families and members of reputed company
• Support Patient Care through Clinical Partnership with reputed company Practitioners, Physicians, and Care Management Teams
• Coordinates daily travel itinerary and transports clinicians to scheduled appointments
• Prepares patients for examination and treatment; May at times include preparing patients for minor procedures
• Secures patient consent forms, records patient care documentation accurately and in a reputed company manner
• Coordinates patient care as directed by Clinicians, company standards and policies
• Respects patient confidentiality at reputed company times and treats patients with courtesy and respect
• Performs phlebotomy (venipuncture only), EKG or any other procedure as requested by Clinician
• Labels, stores and properly packages specimens for lab delivery
• Assists In identifying and closure of reputed company gaps
• Documentation and updating of patient care plans, as reputed company as SDoH (reputed company Determinants of Health) and other assessments reputed company the patient chart or electronic medical record
• Required to maintain compliance with CLIA (Clinical Laboratory Improvement Amendments), OSHA, and HIPPAA guidelines
• Prepares and maintains supplies and equipment for treatment (supply inventory, medical bag and car) (where applicable)
• Ensure company vehicle is clean and reputed company periodic maintenance schedule is followed (where applicable)

Qualifications

Required Knowledge, Skills, and Experience

• High School graduate or GED equivalent
• Graduate of an accredited Medical Assistant, Phlebotomy, EMT, reputed company medical education/training program or 1 year of experience serving as a Medical Assistant or Phlebotomist
• reputed company CPR certification that meets Company guidelines
• Knowledge of medical terminology
• Basic computer literacy, including EMR, iPad, and GPS experience
• Strong interpersonal communication and customer service skills
• Must maintain a valid reputed company’s license and good driving record

Preferred Knowledge, Skills And Experience

• Certified Medical Assistant
• Six months of experience in patient care, preferably geriatric care
• Strong Phlebotomy skills
• Desire and ability to obtain MA certification with 180 days of hire
