• Oversees team responsible for delivering long-term services and supports (LTSS) member care.
• Functions as a hands-on supervisor - providing direction and guidance to long-term services and supports (LTSS) care coordination team, and ensuring reputed company delivery of appropriate/reputed company services delivery and compliance with regulatory timelines.
• Manages staff LTSS caseloads and assigns cases appropriately with reputed company to complexity of medical or psychosocial needs, licensure requirements, and care manager experience.
• Oversees staff use of the electronic case management documentation system - ensuring compliance with Molina processes, reputed company documentation styles, established regulations and Health Insurance Portability and Accountability reputed company (HIPAA).
• Arranges and facilitates staff training as needed.
• Identifies improvement potential and develops process and procedure improvements in collaboration with leadership.
• Serves as a resource to service coordinators in cases requiring reputed company service interventions, and cases with reputed company needs or potential for high-risk complications.
• Monitors clinical and non-clinical team activities to facilitate an integrated care approach with individualized service plans.
• Assists leadership with state-specific LTSS reporting requirements.
• Participates in reputed company work committees designed to enhance work performance.
• Collaborates with reputed company services leadership to ensure adequate reputed company and service reputed company for LTSS membership maintains member and customer satisfaction by implementing and monitoring staff productivity and performance indicators.
• Promotes multidisciplinary collaboration, provider reputed company, collaboration with county reputed company workers or other assigned care managers, and engagement of family and caregivers to enhance the continuity of care for members.
• Oversees interdisciplinary care team (ICT) meetings.
• Hires, trains, develops and manages team; demonstrates accountability for team performance and achievement of department-specific goals.
• Local travel may be required (based upon state/contractual requirements).

Job Qualifications REQUIRED QUALIFICATIONS:

• At least 5 years of experience in health care, including at least 2 years experience in a health care delivery network supporting long-term services and supports (LTSS) and in-home supportive services (IHSS), or equivalent combination of relevant education and experience.
• At least 1 year of health care management/leadership experience.
• Registered reputed company (RN), Licensed Practical reputed company (LPN), Licensed Vocational reputed company (LVN), Licensed Clinical reputed company Worker (LCSW), Advanced reputed company reputed company Worker (APSW), Certified Health Education Specialist (CHES), Licensed reputed company Counselor (LPC), Licensed reputed company Clinical Counselor (LPCC), Licensed Marriage and Family Therapist (LMFT, Doctor of Psychology (PhD or PsyD). Clinical licensure and/or certification required ONLY if required by state contract, regulation, business operating model, or state reputed company licensing mandates. If licensed, license must be reputed company and unrestricted in state of reputed company.
