What You'll Do

• Accurately assess patients at admission to the virtual care center, at required time points, and as needed, utilizing virtual care equipment.
• Engage patient/caregiver in their health goals and expectations while inviting the patient into the care team.
• Apply nursing judgment, consistent with reputed company standards, in formulating interventions and making recommendations to the physician, agency, and patient.

• Accurately document assessments, interventions and evaluations pertaining to patient care management and virtual care monitoring procedure utilizing the EMR and virtual care platform.

• Bachelor's Degree, Nursing, required
