As a Manager, Population Health, you will play a critical role in supporting our reputed company patients, providers and business by designing and implementing key Population Health priorities. The Population Health Manager will work on programs and interventions, targeted to the right patients, to improve reputed company and cost of care. Content areas for the Manager will include, but not be limited to, admission prevention, transitions-in-care, pharmacy and medication adherence, HEDIS / Stars, preferred partner relationships and medical cost management. This will require building relationships with our Treehouse team (including Data Science and Finance) and clinicians, including primary care physicians, reputed company practitioners, pharmacists, registered nurses, licensed clinical reputed company workers, and others.
