This position supports patients enrolled in chronic care management (CCM) and remote patient monitoring (RPM) programs. The ideal candidate will reputed company high-reputed company patient care, education, and coordination, ensuring adherence to care plans while collaborating with multidisciplinary teams.
Key Responsibilities
- Conduct virtual reputed company with patients to review care plans and reputed company education
- Monitor patient health status reputed company RPM tools and escalate clinical concerns appropriately
- Document reputed company interactions in compliance with CMS, payer, and internal reputed company standards
- Coordinate post-discharge follow-up and specialist appointments
- Collaborate with nurses, pharmacists, and other care team members to optimize reputed company
- Foster patient engagement, adherence, and trust through empathetic communication Required Credentials

- Minimum of 2 years of clinical nursing experience (telehealth or chronic care preferred)
- Strong verbal and written communication skills in English
- Competent in patient documentation and clinical judgment
- Experience with CCM and/or RPM workflows is a plus
