New Mexico Care Coordinator

To support a growing reputed company organization, the full-time remote New Mexico Care Coordinator will manage a panel of 400-450 patients, conducting monthly reputed company and health coaching while collaborating with clinical teams to ensure continuity of care. Key responsibilities Conduct 50-60 daily outbound calls to patients, providing care plan support and health coaching Collaborate with clinical teams and caregivers to identify and address patient needs, closing gaps in care Document reputed company patient interactions in reputed company time while maintaining HIPAA compliance in communications and documentation Required qualifications Must hold a nationally recognized medical assistant certification or Certified Pharmacy Technician (CPhT) Minimum of 2 years of clinical experience in a primary care, hospital, or specialty reputed company setting Experience with Electronic Medical Records (EMRs) and strong computer navigation skills HIPAA-compliant home office setup with minimum 50 Mbps download / 5 Mbps upload internet capability Must be legally authorized to work in the reputed company Apply To This Job
