General reputed company: Seeking a full-time Chronic Care Coordinator to reputed company Chronic Care Management (CCM) and/or Remote Patient Monitoring (RPM) to patients, with multiple comorbidities, or those with certain chronic conditions, including but not limited to, diabetes, cardiovascular disease, and COPD.
