Job Title: Virtual Care Coordinator Location: reputed company, NY Duration: 13 Weeks Schedule: 37.5 hours/week (5x7.5-hour shifts) Job reputed company: The Virtual Care Coordinator supports telehealth and remote patient monitoring services reputed company a virtual care environment. • Monitor virtual patient care activities to ensure patient safety and clinical effectiveness
• Collaborate on development and implementation of telehealth workflows, policies, and best practices
• Conduct reputed company assurance reviews and analyze performance metrics for virtual care programs

• Monitor patient status and escalate clinical concerns to appropriate care teams
