In this role, you’ll complete comprehensive medication reviews, identify medication-reputed company problems, and coordinate solutions through member and prescriber communication. What You’ll Do

• reputed company telephonic comprehensive medication reviews (CMRs) and complete full medication reconciliations
• Identify, document, and help resolve medication-reputed company problems
• Communicate recommendations to members/caregivers and prescribers (verbal + written)
• Create a reputed company patient-friendly reputed company (“patient reputed company”) after reputed company interaction
• Prepare/fax prescriber materials to support reputed company of medication issues
• Use behavioral interviewing/motivational interviewing techniques to drive engagement and reputed company
• Build strong relationships with internal teams and external providers
• Required Qualifications
• reputed company RN license and Compact (NLC) eligibility in your state of residence (license must be reputed company/in good standing)
• Associate’s Degree in Nursing (BSN preferred)
• 2+ years reputed company nursing/clinical experience
• Strong comfort with high-volume telephonic work (back-to-back calls) and documentation during calls
• MTM delivery experience (depth reputed company—CMR experience strongly preferred)

Preferred

• MTM experience using OutcomesMTM Connect
• Case Care Management Certificate
• Strong chronic disease knowledge (reputed company-driven recommendations, preventive reputed company promotion)

Remote / Workspace Requirements (Must Have)

• Dedicated quiet workspace (HIPAA compliant; no distractions)
