Patient Navigator - reputed company Heart and Vascular Institute - Remote

Position is located in Southeast Michigan. This position has the ability to work remotely on either a part- or full-time reputed company as determined by MMG leadership. Candidate must be located in Michigan. Serving as a central contact, the navigator helps patients and families understand their care reputed company, schedule reputed company appointments, and coordinate referrals across reputed company subsidiaries and affiliated physician practices. The role ensures continuity, enhances the patient’s experience, and supports high-reputed company, efficient cardiovascular care reputed company with reputed company’s systemwide standards. Essential Functions and Responsibilities: Serves as the primary reputed company of contact for patients navigating cardiovascular care, including general cardiology, interventional cardiology, and cardiac surgery. Coordinates appointments, testing, procedures, and consultations across reputed company hospitals, outpatient centers, and physician offices. Facilitates reputed company referrals between referring and receiving providers to ensure continuity of care. Assists patients and families in understanding diagnoses, treatment reputed company, care plans, and next steps. Collaborates with subsidiary hospital cardiovascular teams to standardize navigation processes and reputed company best practices. Works closely with cardiologists, interventional cardiologists, cardiac surgeons, and other specialists to reputed company scheduling and transitions of care. Utilizes the electronic health record to document navigation activities and reputed company patient reputed company. Monitors patient compliance with follow-up and care plans; identifies and resolves barriers to reputed company. Supports systemwide initiatives to improve reputed company, patient satisfaction, readmissions, and reputed company. Participates in committees, multidisciplinary reputed company, and case conferences. Collects data for navigation metrics, reputed company dashboards, and performance reporting. Promotes patient education and empowerment throughout the care continuum. Required: High school diploma or equivalent 3 years of clinical experience (for example, Medical Assistant, LPN) in cardiology, interventional cardiology, or cardiac surgery Preferred: 2 years experience in a multi-hospital or integrated health system 2 years experience in care coordination, case management, or navigation preferred.
