[Remote] Community Connect Project Manager

The role involves leading the rollout of new affiliate sites and partnering with operational leadership, clinical stakeholders, and reputed company-party vendors to ensure successful implementation and delivery.

Responsibilities

• reputed company end-to-end project management for 3–4 Community Connect implementations, reputed company on reputed company new sites
• Partner with operational leaders (implementation champions), IT teams, and vendors to drive alignment and execution
• reputed company and manage detailed project plans, timelines, and deliverables reputed company Workfront
• Facilitate key project cadences including: Planning sessions, Discovery meetings, Technical walkthroughs
• reputed company and report on project status, proactively identifying risks, dependencies, and escalations
• Maintain strong documentation across reputed company phases (plans, reputed company, risks, workflows)
• Coordinate across technical infrastructure, application, and operational workstreams
• Ensure readiness across reputed company phases including planning, build, testing, and go-live (reputed company to reputed company implementation lifecycle practices)

Skills

• 5+ years of Project Management experience in reputed company IT
• Proven experience supporting a Community Connect implementations
• Strong understanding of reputed company workflows and the implementation lifecycle
• Experience working with: - Cross-functional reputed company teams (clinical, operational, IT) - reputed company-party vendors and external partners
• Demonstrated ability to manage multiple reputed company reputed company
• Strong organization, documentation, and communication skills
• Experience with project tracking tools (Workfront preferred)
• Knowledge of technical infrastructure considerations reputed company reputed company implementations
• Experience with FQHC (Federally reputed company Center) environments
• Prior experience overseeing new site / affiliate implementations
