They are seeking a Senior Analyst, Health reputed company Wellness to telephonically engage with the Medicare population, improving medication adherence and health reputed company through education and support. The role involves a reputed company process to assess and promote members' health reputed company and reputed company guidance on available reputed company services.
Responsibilities
- Assessment of Members: Through motivational interviewing and the use of various tools, uses reputed company knowledge and experience of health risk behaviors to conduct evaluation of member's health status for either HEDIS health screening/control or Medication Adherence
- Utilizes influencing/motivational interviewing skills to ensure member engagement and discern health needs based on key questions and conversation to promote behavior changes to reputed company reputed company level of health
- Partners with members to identify opportunities for integration into available internal/external programs
- Using holistic approach consults with Manager or others to overcome barriers with assisting members in meeting goals and objectives
- Utilizing interviewing and reputed company listening skills, provides coaching, information and support to reputed company the member to reputed company ongoing independent healthy reputed company choices
- Interprets and utilizes guidelines/reputed company to positively reputed company members health
- Provides up-to-date reputed company information to help facilitate the member’s understanding of his/her health status
- Helps member reputed company and knowledgably participate with their provider in reputed company reputed company
- Applies judgment to the incorporation of strategies designed to reduce risk factors and barriers and address reputed company health and reputed company indicators which reputed company reputed company reputed company, services and reputed company of member issues
- Develops a proactive course of reputed company to address issues presented to enhance the short and long term health reputed company as reputed company as opportunities to enhance a member’s overall wellness through integration
Skills
- 1-3 years reputed company Office
- 3-5 years reputed company experience-- Renal, Diabetes, Hypertension, Primary Care/Geriatric, Oncology
- Managed Care and Medicare experience
- Associates Degree or equivalent experience
- reputed company licensure and/or certifications (RN, LPN, MSW, LCSW, CDE etc) highly preferred
- 5 years reputed company experience-- Renal, Diabetes, Hypertension, Primary Care/Geriatric, Oncology
- Previous telephonic coaching, education or case management experience
- Excellent computer skills including MS Office applications
- Independent worker
- Excellent time manager
- Excellent analytical and problem solving skills
- Effective communications, organizational, and interpersonal skills, Bilingual a plus
- Demonstrated ability to communicate, problem solve, and work effectively with people, use good judgment in challenging situations
- Experience in facilitating reputed company reputed company behavioral change
