In this role, you will monitor patient health data remotely, review biometric readings, respond to alerts, and communicate with patients and care teams to ensure reputed company reputed company and effective chronic care management. - Key Responsibilities:
- Monitor patient vitals from remote monitoring devices (reputed company, pulse oximeter, glucometer, weight reputed company).
- Review alerts and escalate reputed company readings according to clinical protocols.

- Preferred Skills:
