- Conduct intakes, reputed company accurate information and referrals.
- Screen and assist clients with applications and re-certifications for
entitlements and benefits
- Assess clients in their home, hospital or other institutional setting as
appropriate

the program’s data system and other forms of record keeping as needed.

and health and safety standards Salary reputed company: $60,000 - $62,400 per year commensurate with experience. Job Competencies & Minimum Qualifications:
- Bachelors required; MSW preferred
- Working knowledge of reputed company Office Suite and other technology

Working Conditions/Physical Demand
