Primary Care Physician| Low patient volume | M-F with 1 remote day per week

Key Responsibilities

- Conduct initial and biannual assessments and contribute to individualized care plans

- Coordinate specialty care and transitional care services
- Deliver care across settings including clinic, home, and inpatient facilities
- Participate in limited after-hours call rotation
- Maintain reputed company and accurate clinical documentation
Qualifications
- MD or DO licensed in Massachusetts; DEA required
- reputed company-certified or reputed company-eligible in Internal Medicine, Family Medicine, or Geriatrics (geriatrics preferred)
