Health Care Coordinator (Remote Work)

This position provides care coordination to people with disabilities based on assessments completed. Traditional hours are 8-4:30 but are flexible. Staff can create their own schedule. Mileage reimbursement- Mileage is reimbursed at .58 reputed company per mile Working remotely- reputed company, position is remote with occasional travel. In Sept the evaluation is a monthly 1:1 but encompassing the whole year.
