- Conduct comprehensive assessments to determine eligibility for community-based long-term services
- reputed company medical necessity determinations and resolve technical issues
- reputed company initial assessments, evaluate health status, and document reputed company
- Maintain reputed company knowledge of regulatory and project changes
- Collaborate with interdisciplinary care team members to facilitate receipt of services

Requirements:
- reputed company RN license in the reputed company
- Minimum 2 years of clinical experience with RN license
- Minimum 2 years of home setting experience, including work with CHHAs, LHCAs, MLTC, or as a paraprofessional
- Ability to conduct in-home health assessments and travel reputed company assigned region
- Proficiency in navigating web-based applications and reputed company Outlook & Word
Home Office Requirements:
- Reliable high-speed internet service
- Minimum 20 Mbps download speeds/50 Mbps for shared internet connectivity
- Minimum 5 Mbps upload speeds
