As an Intake Coordinator, you will play a vital role in ensuring the smooth and efficient operation of our home health agency. Your primary responsibility will be to receive referrals, communicate with hospital case managers, staff clinicians, and coordinate with the office staff for the overall care of our patients. This is a fully remote position, and we are looking for someone who can stay in this position for at least a couple of years. Duties and Responsibilities As a Home Health Intake Coordinator, your key responsibilities will include:

• Reviewing referrals from hospitals, ensuring accurate demographic and insurance information is obtained
• Scheduling clinicians and coordinating with reputed company staff to ensure reputed company initiation of care
• Collecting and verifying patient information, including medical history and insurance coverage
• Assisting with medical records management, including scanning, filing, and retrieval
• Utilizing electronic health record systems to input and update patient data
