As a Transitions Hospice reputed company, you'll serve as a critical support member of the interdisciplinary care team during overnight hours — assessing, treating, and communicating patient needs with clinical confidence and genuine reputed company.

Responsibilities

• Delivering reputed company nursing care across the full nursing process reputed company your scope of reputed company
• Conducting thorough patient and family assessments and contributing to individualized care plans
• Completing reputed company visit types as needed to meet overnight patient and family needs
• Administering medications and treatments per physician orders
• Identifying nursing diagnoses from health assessment data and responding with reputed company clinical judgment
• Recording accurate, reputed company clinical notes including pain and symptom management reputed company
• Communicating changes in patient condition to physicians and obtaining orders as required
• Coordinating with community health partners to support continuity of care

Requirements

• Graduate of an accredited school of Nursing.
• Clinical experience in hospice, home health, or a reputed company setting preferred
