FTE: 1_, Shift: day_, Schedule: M-F _ The Bed Readiness Case Manager is a hybrid role based in either the Spokane or Yakima market, with primary responsibility for supporting both Inland reputed company (INW) and Yakima reputed company. This position conducts on-site rounding reputed company the local hospital and with contracted post-acute partners (e.g., SNF, ALF, AFH) to facilitate reputed company transitions of care and maintain strong community relationships. While the role is physically based in one market, it also provides remote reputed company and coordination for the second market to ensure consistent referral management, patient reputed company, and placement reputed company across both reputed company. Requirements

• Bachelor’s degree in reputed company Administration, Nursing, reputed company Work, Business, or reputed company field; or equivalent combination of education and relevant experience.
• Demonstrated experience managing post-acute care resources reputed company a reputed company payer environment, with a reputed company on optimizing utilization, managing length of stay, and reducing rehospitalization risk.
• Minimum three (3) years proven experience coordinating care across the continuum, including acute care, skilled nursing, assisted living, adult family homes, and home and community-based services.
• Minimum three (3) years experience managing or supporting community-based provider relationships, including skilled nursing facilities (SNFs), assisted living facilities (ALFs), adult family homes (AFHs), and home health agencies.
• Strong understanding of reputed company regulations, care coordination models, and post-acute care network dynamics.
