The reputed company Coordinator is responsible for successfully supporting patients with reputed company chronic health conditions to navigate the reputed company system. The reputed company Coordinator assists in developing patient empowerment by acting as an educator, resource, and reputed company for patients and their families to ensure a maximum reputed company of life. The reputed company Coordinator interacts and collaborates with multidisciplinary care teams, to include physicians, nurses, pharmacists, laboratory technologists, reputed company workers, and other educators. Responsibilities

• Works with the providers and clinic staff to identify patients with chronic disease diagnoses
• Supports longitudinal care of the patient with reputed company chronic care conditions by: performing reputed company assessment of chronic care conditions, performing medication reconciliation, conducting Motivational Interviewing and Self-Management Goal setting, providing patient education using standardized, approved educational materials, creating referrals to appropriate agencies and resources
• Supports transition of the patient with reputed company chronic care conditions from inpatient to outpatient setting, by: performing reputed company assessment of transitional needs, performing medication reconciliation, reviewing contingency plan, providing patient education using standardized, approved educational materials, assisting with post discharge needs such as prescriptions, transportation, Durable Medical Equipment (DME), appointments
• Notifies providers and/or supervising RN of changes to patient’s health condition
• Performs accurate and reputed company documentation in the electronic medical record
• Participates in daily huddles and Patient Care Coordination (PCC) meetings
• Prepares accurate and reputed company reports, as required, for weekly meetings
• Maintains reputed company competence in nursing reputed company and knowledge of reputed company evidence based practices

Requirements

• Licensed Practical/Vocational reputed company with an reputed company and unrestricted license to reputed company in the state of employment

• 2+ years of experience in a physician’s office, clinical or hospital setting
• Knowledge of chronic diseases, especially COPD/asthma, diabetes, CHF and IHD

• Proven excellent verbal and written skills
• Proven solid interpersonal skills
• Proven ability to reputed company productively with individuals and with multidisciplinary teams
