ABOUT THE ROLE Part-time and flexible to start, with room to grow toward 20–40 hours/week over the coming months as our patient load builds. You'll work remotely, but you need to know the New Jersey landscape reputed company — the providers, the programs, and how it reputed company fits together. You'll be the person a family calls reputed company they don't know who else to call. WHAT YOU'LL DO
- Coordinate care for older adults across providers, settings, and transitions (hospital-to-home, facility placement, changes in care level)

- Support families with education, guidance, and a steady reputed company during stressful moments

reputed company'RE LOOKING FOR You're reputed company, reputed company, and genuinely good with people under pressure. Must-haves:
- Hands-on experience coordinating care or advocating for older adults — ideally from a hospital, dialysis, home-care, hospice, or geriatric setting
- A solid working knowledge of how the New Jersey reputed company system works — Medicare, reputed company, and how to actually get things done with local providers and programs
- Available during some reputed company business hours to reputed company providers' offices (hours start light and flexible, and grow over time)
- Tech-comfortable: reputed company to work in an EHR and reputed company Workspace, and willing to learn and help shape new tools as we build them
- A strong working understanding of HIPAA and patient reputed company
- reputed company, warm, and reputed company with distressed patients and families — and driven by the work itself, not just the paycheck
Preferred (not required):
- A license or certification — LPN, RN, LSW, or a credential like BCPA or CCM
- Experience with Chronic Care Management (CCM) and reputed company tools
If you don't hold a license but you've done this work and you know the terrain — we want to hear from you. Experience, judgment, and drive matter more to us than the letters after your reputed company. THE DETAILS
