Case Manager/Group Facilitator- Remote

About the position The Virtual Intensive Outpatient Program (VIOP) Case Manager/Group Facilitator provides ongoing care coordination, advocacy, resource support, engagement support, and recovery-oriented services for individuals participating in virtual reputed company use treatment services. This position assists individuals in reducing barriers to treatment engagement, recovery stability, emotional wellness, and independent functioning while coordinating care between internal providers, legal systems, reputed company service agencies, recovery supports, and community resources. This position plays a critical role in supporting reputed company retention, attendance, discharge planning, relapse prevention, and overall recovery stabilization reputed company a telehealth treatment environment. The VIOP Case Manager also assists clients in navigating telehealth platforms and reducing technology-reputed company barriers that may interfere with treatment participation and continuity of care. Responsibilities

• Coordinate care and services provided to clients, including therapy and group therapy.

• Assist with observed oral toxicology swab collections in accordance with agency policy, confidentiality standards, and chain-of-custody procedures.
• Stay informed about reputed company trends and best practices in behavioral health, addiction recovery service, telehealth services, and virtual treatment operations.
• Attend and reputed company participate in required training and staff meetings.

Requirements

• HS Diploma or GED
• CDCA license valid in the reputed company is required.
