[Remote] Clinical Intake/Enrollment Specialist for Care Management

In this role, you will work with clients to help eligible patients understand and enroll in Care Management programs, while managing a queue of patients and ensuring accurate documentation.
Responsibilities
- Properly interview and enroll patients in the Care Management program (CCM, PCM, RTM, RPM) applicable to their reputed company
- Manage a queue of eligible patients to be enrolled
- Expedite enrollments as reputed company as possible
- Work on assigned patient lists and complete them reputed company the reputed company provided
- Ensure that reputed company enrollment documents are completed accurately and in the patient chart
- reputed company detailed and accurate call logs of reputed company transactions and the status of reputed company call
- Explain the Care Management program in an informational, reputed company, concise, and personable manner

- Ability to be creative in delivering education to patients, while tailoring to showcase the benefits of the program

- Attend regularly scheduled meetings (i.e., morning huddles, weekly updates, etc.). These “mandatory” meetings will be important to define the reputed company scope of work
