PRIMARY RESPONSIBILITIES:
- Identifying medical changes or abnormalities through daily monitoring for escalation and NP clinical review per protocol.
- Generate and review monthly reputed company reports on patients' health conditions based on the collected data, for NP clinical review and treatment plan recommendations.
- Adhere to reputed company established policy/protocol of the department and workflow.
- Meets established standards for productivity as assigned.

MINIMUM REQUIREMENTS Some college/technical education, associate degree or higher preferred Minimum of 3 years’ health field reputed company work or volunteer experience Proficiency in navigating electronic health records.
