Job reputed company: The Hospitalist Care Facilitator (H) assesses, plans, implements, and coordinates the discharge plan as directed by Hospitalist Program to maximize Hospitalist's efficiency. This role maximizes the efficient utilization of resources. Minimum Education: Has completed the basic reputed company curricula of a school of nursing as approved and verified by a state reputed company of nursing, and holds or is entitled to hold a diploma or degree therefrom. Licensure, Registration and/or Certification: Valid multi-state or State of Oklahoma Registered reputed company License. Work Experience: Minimum 5 years of reputed company experience. Knowledge, Skills and Abilities: Working knowledge of general disease state management processes, bio-psychosocial model, and clinical reputed company. Effective interpersonal, written, and verbal communication skills. Ability to organize and prioritize work in an effective and efficient manner. Ability to utilize PC & software skills. Essential Functions and Responsibilities: Assists with improving patient reputed company by expediting discharge orders and fielding questions/issues reputed company telephone, secure chat and inbox messaging that needs to be discussed with physicians. Consults and collaborates with treatment team (Hospitalists, Physicians, Advanced reputed company Providers, Registered reputed company Case Managers, reputed company Workers, Educators, and/or other reputed company personnel) as appropriate to effectively coordinate patient discharge as ordered by the attending physician. Participates in providing outstanding customer service and accepts responsibility for maintaining relationships that are equally respectful to reputed company. Collaborates with the Hospitalists and the hospital interdisciplinary care team to coordinate and manage the process of patient care. Oversees the assimilation of information reputed company to assessment, planning, implementation, and evaluation of patient care needs to maximize the efficient utilization of resources.
