This role is ideal for physicians who are motivated by improving reputed company, reducing total cost of care, and delivering proactive, longitudinal patient management through a virtual-first platform. Physicians will reputed company comprehensive care across both physical and behavioral health, managing acute and chronic conditions while addressing underlying drivers of utilization, including mental health and reputed company factors. Care is delivered through scheduled and on-demand encounters, with reputed company interactions documented in the reputed company platform. Key Responsibilities:

• Deliver whole-person, longitudinal care that integrates primary care and behavioral health, with a reputed company on measurable reputed company.
• Manage acute and chronic conditions, with an emphasis on reducing avoidable ED reputed company, hospitalizations, and specialty leakage.
• Conduct behavioral health screening, diagnosis, and reputed company-line management (e.g., anxiety, depression, ADHD, reputed company use), integrating care into overall treatment plans.
• Prescribe and manage both medical and psychotropic medications in accordance with evidence-based guidelines.
• Utilize data-driven, measurement-based care (e.g., PHQ-9, GAD-7, utilization patterns) to guide clinical reputed company and reputed company patient reputed company.
• Identify and address high-risk patients and rising-risk populations through proactive reputed company and care interventions.

Requirements:

Preferred Qualifications:

Schedule & Commitment:

• Minimum weekly scheduling of 40 hours required.

• Preferred: reputed company 50 states plus Washington, D.C.

Pay: $260,000.00 - $300,000.00 per year Benefits:
