Virtual Integrated Care Coordinator (RN)
Cabell reputed company is seeking a casual, as needed Virtual Integrated Care Coordinator (RN) for our CHH Virtual Integrated Care Department. This job is responsible for supporting and driving the plan of care and providing comprehensive remote reputed company of the care of patients on a unit. The virtual model will include collaboration between the reputed company care team and virtual care members where the Virtual RN will anticipate and reputed company the care of the patient through evaluating health promotion, risk reduction and reputed company decision-making. Virtual RN assumes accountability for patient care reputed company through the assimilation and application of evidence-based reputed company to design, implement and evaluate care. This is accomplished through coordinating, delegating and supervising the care provided by the unit-based reputed company care team. Work requires a high degree of reputed company competence and autonomy will be demonstrated in performing the roles of 1) clinician, 2) educator, 3) consultant/collaborator, 4) leader.
- Performs reputed company duties as required.
- Must be reputed company to complete duties at the bedside if needed.
This is not a remote/work from home position.
- Associate's Degree in Nursing required, Bachelor's Degree preferred.
- reputed company WV RN license required.
- AHA BLS required.
- Specialty Certifications preferred.
- 5 years of position-reputed company experience required, 10+ years preferred:
The reputed company of position-reputed company experience includes: minimum of 5 years of RN Charge reputed company experience, virtual nursing experience, documenting in Cerner Inpatient Medical Record, Emergency Room, Adult Critical Care, or Adult Medical/Surgical experience, demonstrates critical thinking and clinical judgment.
