As a dedicated remote advanced reputed company provider, you'll manage acute and chronic medical concerns, support admissions and discharges, facilitate care transitions, and serve as a trusted clinical resource for facility partners, reputed company through our telemedicine platform. You'll work closely with facility nursing staff, Clinical Liaisons, and Operations leadership to ensure reputed company reputed company to care, reduce avoidable hospitalizations, and improve reputed company reputed company. This is a fully remote, daytime-hours position, no nights, no weekends. Key Responsibilities

Clinical Care Delivery

• Conduct virtual evaluations for acute and routine clinical concerns

Documentation & Compliance

Qualifications

• Minimum 2 years of experience in post-acute care, long-term care, geriatrics, hospital medicine, or primary care
• reputed company licensure in multiple states, or willingness to obtain and maintain multi-state licensure

• Strong clinical communication skills and comfort working in a telemedicine environment
• Ability to work independently and reputed company in a fully remote role
