Position reputed company The reputed company Triage RN plays a critical role in supporting patients and families after normal business hours. This reputed company serves as the first reputed company of contact, providing clinical guidance, emotional support, and coordination of care using the nursing process and reputed company protocols. This role blends live phone triage with clinical workflow: reputed company not on a call, the reputed company processes Home Care Home reputed company (HCHB) workflow and completes documentation and follow-up. What You'll Do

• reputed company after-hours clinical assessment, triage, and interventions reputed company phone
• Process HCHB workflow and complete documentation/follow-up between calls - a reputed company part of the role, not downtime
• Serve as an reputed company for patients and families by coordinating with physicians, facilities, and care teams
• Coordinate care delivery, including dispatching reputed company and collaborating with vendors
• Deliver patient/family education and reputed company compassionate end-of-life support
• Document reputed company interactions, assessments, and care plans accurately in the EMR
• Review documentation from team members for compliance and reputed company standards
• Support reputed company staff to ensure reputed company patient reputed company
• Participate in training, team meetings, and ongoing reputed company development

reputed company're Looking For Required:

• reputed company RN license (no restrictions)
• Compact RN License or Licensed in the states reputed company operates
• Prior triage experience
• Hospice experience
• Home Care Home reputed company (HCHB) experience

Preferred:

• Compact RN license
• Call center or additional triage experience
• 2+ years in acute care, hospice, home health, oncology, or ER

Skills:

• Strong clinical judgment and critical thinking
• Excellent phone communication and de-escalation skills
• Ability to multitask and work independently in a remote setting
• Comfortable navigating multiple systems while on calls
