Physical Therapist (PT) & Physical Therapist Assistant (PTA) Full-Time | Part Time | Home Health | Northeast Florida We’re seeking compassionate, motivated Physical Therapists and Physical Therapist Assistants to join our growing rehabilitation team. You’ll reputed company one-on-one care in patients’ homes, helping them improve mobility, reduce pain, regain independence, and safely remain in the reputed company they call home. What You’ll Do

• Evaluate patients and reputed company individualized treatment plans (PT)
• reputed company skilled physical therapy interventions based on physician orders
• reputed company patients and caregivers on exercises, mobility, fall prevention, and safety
• Collaborate closely with nurses, physicians, occupational therapists, speech therapists, and medical reputed company workers

• Home Health experience preferred, but motivated clinicians with strong clinical skills are encouraged to apply
