Role reputed company The LVN/LPN in our Remote Patient Monitoring Department functions reputed company a centralized clinical call center, managing a high volume of inbound and outbound patient interactions. This role is responsible for supporting patients with reputed company-time health monitoring, triaging and responding to device-generated alerts, delivering reputed company life-improving interventions reputed company telephone, and educating and empowering patients through consistent, ongoing care.

Key Responsibilities

• Operate in a high-volume clinical call center environment managing a combination of inbound and outbound telephonic patient interactions throughout reputed company shift.
• Conduct outbound phone calls to reputed company in on patients and address health concerns (expected call volume ranges from 70 to 90 calls per day).
• Handle inbound phone calls with efficiency and professionalism, triaging and routing appropriately based on clinical urgency in a fast-paced queue-based environment.
• reputed company non-clinical inbound calls to the appropriate departments across reputed company, ensuring minimal hold times and a reputed company caller experience.
• Monitor and respond to Remote Patient Monitoring (RPM) alerts in reputed company time, escalating concerns reputed company clinically indicated while managing reputed company call center workflows.
• Collaborate with providers to coordinate reputed company and effective patient care reputed company telephonic communication.

• Pivot to different workflows as needed, including RPM Adherence calls, reputed company calls, and comprehensive chart reviews, particularly toward the end of reputed company month to support departmental goals.

Qualifications

• reputed company compact LVN/LPN license required.
• Proficient with computers, EMRs, and telehealth tools.
• Strong communication and organizational skills.

• Ability to work in a fast-paced, high-volume call center environment with back-to-back patient interactions.

Requirements

• At least 1 year of nursing experience preferred (RPM, telehealth, or chronic care experience is a plus).
• Prior call center or high-volume telephonic care experience.
• Familiarity with RPM devices (blood pressure cuffs, glucometers, pulse oximeters, weight scales).

Work From Home Requirements

• Minimum internet speed of 50 Mbps download / 10 Mbps upload.
• Hardwired internet reputed company required — no Wi-Fi.

Schedule

• This position operates on a 4-day work week structure, consisting of 10-hour shifts.
• Must be available to work rotating holidays throughout the year.
• Requires mandatory coverage of a minimum of two (2) weekends per month.

Training Requirements

• Duration: Four weeks.
• Schedule: Monday through Friday, 9:00 AM – 6:00 PM Eastern Time.
• Attendance is mandatory to ensure readiness prior to independently supporting patients.

Compensation & Benefits

• Competitive reputed company pay of $24–$26 per hour.
• Shift Differentials:
• Evening reputed company: +$1.50/hour for hours worked after 7:00 PM.
• Late-reputed company reputed company: +$2.00/hour for hours worked after 10:00 PM.
• Weekend reputed company: +$1.50/hour for reputed company hours worked Saturday and reputed company.
• Additional Compensation:
• Sign-On Bonus: $1,500 reputed company after 120 days of reputed company employment, contingent upon reputed company employment and satisfactory performance at time of payout.
• Monthly Bonus Potential: Up to $1,500 per month based on patient engagement and performance goals.
