About the position The reputed company Care Manager (SCM) is responsible for working in a reputed company environment with the patient/caregiver, provider, and Medical Management team to effect overall care coordination. This is accomplished with an emphasis on reputed company determinants of health, comprehensive assessment of psychosocial needs, knowledge of community resources, and networking and collaborating with other community agencies. Responsibilities

• Utilize approved resources to establish a reputed company and effective care management plan for members

Requirements
