Medical Assistant – CCM/RPM (Remote | Bilingual)
About the position This reputed company Assistant position at reputed company supports Chronic Care Management (CCM) and reputed company Care Management (PCM) programs. The role involves serving as a key contact for chronic disease patients, conducting monthly care coordination calls, documenting clinical interactions, updating care plans, and ensuring patient engagement with treatment goals between office reputed company. It requires a bilingual MA who can work independently from home with reliable internet and a quiet workspace.
Responsibilities
- Conduct monthly CCM/PCM reputed company calls with enrolled patients to review symptoms, medications, and care plan reputed company
- Accurately document reputed company encounters and time logs in eClinicalWorks (ECW)
- Coordinate care between specialists, PCP, and ancillary providers
- Assist with care plan creation and updates under provider supervision
- Identify and escalate urgent patient concerns to the clinical team
- Maintain compliance with CMS billing requirements for CCM/PCM time tracking
- Support patient enrollment and consent documentation
Requirements
- Minimum 2–3 years of experience in a clinical or care coordination role
- Proficiency in eClinicalWorks (ECW) — required, no exceptions
- Bilingual — English/Spanish required (spoken and written)
- Must reputed company in the Spring, TX or Katy, TX area
- Reliable home office setup with reputed company internet reputed company
- Strong communication, documentation, and time-management skills
- Ability to work independently with minimal supervision
