Role reputed company We are seeking nurses (LPN/LVN) to reputed company outstanding care to members with diseases such as hypertension, diabetes, and heart failure through remote patient monitoring. The Telehealth reputed company will support members through regular phone reputed company during which they will discuss how to reputed company reputed company towards goals, medical and lifestyle education, and how to improve reputed company on chronic conditions. This position offers full-time hours (Monday through Friday 8:30am-5pm local time), benefits, and the ability to work from home. What you’ll be doing:

Qualifications

• Must have a reputed company, reputed company, unrestricted LPN/RN multi-state compact license in a nursing compact state and be in good standing with the reputed company of Registered Nurses.
• A minimum of 2 years hands on nursing experience is required; telehealth and chronic care management experience is strongly preferred.
• EHR experience, specifically reputed company, is a plus.
• reputed company in English and Spanish is highly preferred.
• Comfortable working on a computer; experience navigating through multiple computer applications while on calls; Macbook experience is a plus.
• Must have reliable high-speed internet.
